For an annual fee, you can receive all the dental care you need at an affordable price. The North Forest Family Dental Care dental plan coverage includes:
- 2 dental exams per year
- 1 limited exam per year
- 2 general dental cleanings per year
- 1 full mouth series treatment per every 5 years
- 1 adult bitewing x-ray per year
- 2 child bitewing x-rays per year
- 2 child fluoride treatments per year
- 4 periodontal maintenance treatments per year (with an additional periodontal treatment plan)
- 20% off all other dental procedures (excluding orthodontic treatment)
Yearly membership dues
- 1st member – $299
- Each additional member – $249
All family members involved in this plan must have the same address.
Terms and Limitations
- One family member may not be substituted for another.
- This plan is non-refundable. You cannot receive a refund if you choose not to use this plan.
- This is a discount plan, and is not valid with any other dental insurance.
- Your effective date is the day you sign up. Your renewal date is the same date every year.
- Co-payments are due at the time of service.
- Rates are subject to change annually.
- This plan is only valid at North Forest Family Dental Care.
